*Note: If you want genetic test results to be included on an HSS registration or transfer paper, you will need to submit a copy (photo OK) of the results from a nationally recognized lab along with the application.

How to use these Fillable Application forms:

To fill out electronically via the .PDF form:

1. Download the relevant PDF forms to your computer.

2. Create and name a copy for each animal or embryo group which you have.

3. Open the PDF in a modern, up-to-date browser, or in Mac Preview, or in Acrobat Reader.

4. Fill out all necessary fields by clicking or tabbing through them like a typical web form, then save and close the file.

5. Repeat for each form as needed. (Then open each again to double check that all information is correct.)

6. Attach all of the PDF files to an email and send to registration@heritageshorthorn.org along with a filled out Order Form/Fee Schedule. Please note which payment method you prefer. (See note below on how to make the actual payment.)

To fill out via paper or handwritten or taking a photo of completed Application form:

Note: There is a $15 surcharge for each mailed paper application or for emailed photos of an application because of the extra time it entails to manually transcribe the information into our database.

1. Download the relevant PDF forms to your computer.

2. Print out however many copies you need.

3. Fill them out, including an Order Form/Fee Schedule and mail them to: Heritage Shorthorn Society, 6515 Kurtz Rd., Dallas, Oregon 97338), or take smart phone pictures of the completed forms and send them as email attachments to HSS office.

4. Either include a check for the full amount if you are sending the applications in the mail, or be sure to tick the box for Pay Online if you prefer credit card. (See note below on how to make the actual payment.)


Payments:

(Please note that no applications will be processed until payment in full has been received.)

We offer two payment options: Electronic Internet Payment, or Paper Check.

1. For an Internet Payment you can either add multiple items from the Payments Page to your shopping cart to pay in full when you are sending the application forms, or we can email you an invoice with the total amount due after we have received your applications packet. You would then click on the button below, then scroll to bottom payment page and click on “Custom Amount Total” button for making the payment through Stripe, our secure payment processor.

2. If you prefer to mail a check, send it to the following address: Heritage Shorthorn Society, 6515 Kurtz Rd., Dallas Oregon 97338.
